Chuck W. Lewis of Plainfield died Tuesday. He was 55.
He was born Nov. 4, 1957, in Indianapolis.
Mr. Lewis had worked as manager of information technology for Lee Supply Corp. since 1997.
He was a member of Kingsway Christian Church.
Survivors include his wife, Lisa W. (Sparks) Lewis; sons Jonathan (Shannon) and Samuel Lewis; daughters Allie (Josh) Reeves and Anna (Rob) Barr; his mother, LaVelda Brown Lewis; a sister, Sue Lewis; and grandchildren Will and Wes Lewis, Evie Reeves, and Elliot Barr.
Preceding him in death was his father, Samuel W. Lewis.
Services are 1 p.m. Friday at Hampton-Gentry Funeral Home in Plainfield. Entombment will be at Maple Hill Cemetery, also in Plainfield.
Calling is for two hours prior to the service at the funeral home.
